How do I get an admission application for the College of Health Sciences?
To obtain an Admission Application and a Health History Form contact
the admission coordinator, Carole Moores, or click HERE for a printable admission
application form.
Since I filled out the admission application
for the College of Health Sciences, do I need to fill out an application
to be admitted to the University?
Yes! All prospective students must be admitted to Eastern Kentucky University
through the Admissions Office before you can be considered for admission
to a specific program in the College of Health Sciences. University admission
forms are available from the Admissions Office, 203 Jones Building, Richmond,
KY 40475-3101; telephone (859) 622-2106 or visit the Division of Admissions'
homepage for a printable admissions application form.
Since I mailed in the admission application
to the College of Health Sciences, am I now officially admitted into the
program?
No! Due to enrollment limitations of programs, applications should be
made early. Applications received after the deadline of a program will be
processed and admissions recommended on a space available basis. To learn
more about the specific entrance requirements and deadlines for each program
contact that major departmental office. The applicant is responsible for
obtaining information for the application process including program deadlines.
All applicants must provide evidence of their ability to meet the academic
standards established by the various programs.
Will I be considered an entering freshmen, a transfer student, or a readmitted student?

How do I transfer from another university or college to Eastern Kentucky University?
All applicants must submit official transcripts of work from all regional
accredited institutions attended even if no credit was awarded. Transfer
work must be from an accredited institution and with a minimum grade of "C" in
major and supporting courses. The applicant must be eligible to return
to the last institution attended.
What is an official transcript?
An official transcript is defined as having the official seal of the
university and the signature of the registrar. The transcript must be sent
from the Registrar of the applicant's institution to Eastern Kentucky University,
Division of Admissions, 203 Jones Building, Richmond, KY 40475-3101. No hand
carried transcripts accepted!
Will all of my transfer courses be accepted by the College of Health Sciences and Eastern Kentucky University?
The Office of Academic Support and Undergraduate Studies and the academic
department of your major will determine the applicability of your courses
to your academic program.
I have college credit taken at an accredited college or university from quite a few years ago.
Will that credit still be used?
Work taken more than eight years previously must be approved by the Dean
of the College.
What is student liability (malpractice) insurance?
All applicants who are accepted into a College of Health Sciences program
that requires participation in clinical learning experiences must purchase
and maintain liability insurance through Eastern Kentucky University during
the entire clinical experience. Proof of such insurance coverage must be
furnished to the department before the clinical experience is scheduled to
begin.
Where do I purchase the liability insurance and how much does it cost?
The University has arranged for appropriate insurance coverage at a modest
cost to the applicant. Further information may be obtained at the applicant's
major departmental office.
How do I sign-up for student liability insurance?
Students enrolled in a fieldwork or clinical course will register for
the student liability insurance at the same time as you register for classes.
This will generate the fee as part of your tuition/fees for the semester.
Not registering or paying the fee for this will result in disenrollment from
the clinical/fieldwork course(s).
I was not accepted into the program of my choice. What do I do now?
In the event that an applicant is not accepted for a program, the applicant
who wishes to reapply must submit a new application and related materials
that will be considered for the following semester. Contact the department
offering the program for further details.
