Application for Graduation

PLEASE FOLLOW ALL THE STEPS BELOW - WHEN YOU ARE FINISHED YOU SHOULD HAVE THE FOLLOWING ITEMS TO TURN IN:

1) Graduation Application (2 pages)

2) Alumi paperwork (2 pages)

3) Signature page of Survey completion (1 page)

1) Graduation Application	Print out Graduation application by clicking on the box to your left. After you have printed out the graduation application, fill out both pages completely. An incomplete application will not be processed and will be returned to you. Your name will appear on your diploma as indicated on your application. Your diploma will be mailed to the permanent address as indicated on your application.
2) Alumni Survey	Print and complete the EKU Alumni Association/EKU Activities and Organizations Questionnaire in the box to your left (two pages).
Undergraduate Fee Options	If you would like to pay your graduation fee at this time, go to the cashier's window of Billings and Collections, Room 210 in the Student Services Building (SSB). If graduation fees were paid, you will need to turn in your receipt with your graduation application packet, otherwise we will assume you did not pay the graduation fee and your account will be charged. (If you are applying for a certificate only {with the exception of a Post-Baccalaureate}, you do not pay the graduation fee.)
3) Online Survey of Undergraduate Degree Candidates	Complete the Online Survey of Undergraduate Degree Candidates. When you have finished the anonymous survey and submitted it, you will receive a Completion Verification Form. You must print the Completion Verification Form out and include it with your graduation application packet. To take the Online Survey of Undergraduate Degree Candidates, click the link to your left to get to the survey.
IMPORTANT Majors with Education Options	*Majors with Education Options:* (Press on highlighted Links below to go directly to additional on line surveys you will need to take.) Complete the Professional Education Technology Questionnaire and the Professional Education Survey Questionnaire.

Please make sure you have ALL the following items together and the information is complete then please return the packet to the appropriate person in the Dean's office in charge of your program listed below:

Graduation Fee Receipt (if applicable)
Online Survey of Undergraduate Degree Candidates Completion Verification Form
