Preparing for graduation is a hectic time of year for any student. The College of Health Sciences has provided a simple step-by-step procedure for graduating students from this College to use. This procedure will help the future graduate prepare ALL the paperwork needed for graduation. Please read the following directions very carefully.

When you complete the procedure listed below, you will be ready to submit those items as your Graduation Application Packet.

PROCEDURE for Completing Your Graduation Application Packet

Steps 1 – 3 must be completed for the application to be processed.

1. Graduation Application
   This can be downloaded as a [PDF] or [DOC]. If you are having trouble viewing/printing either one of these files, please visit the Downloads page for help.
   You will need to print the Graduation Application and then fill it out completely. An incomplete application will not be processed and will be returned to you. Your name will appear on your diploma as indicated on your application. Also, your diploma will be mailed to the permanent address as indicated on your application.
   
   
2. Alumni Association Information Sheet
   This can be downloaded as a [PDF] If you are having trouble viewing/printing this file, please visit the Downloads page for help.
   You will need to print the Alumni Association Information Sheet and then fill it out completely.
   
   
3. Survey Completion Page
   Disable your pop-up blocker if you are having trouble accessing any of the links below. Also, if a page appears stating "There is a problem with this website's security certificate", that is normal, don't be alarmed. It is just a safety measure your computer uses to ensure that it is communicating with the correct website. Please click "Continue to this website (not recommended)".
   When you have finished the anonymous survey and submitted it, you will receive a Completion Verification Form. You must print the Completion Verification Form for each survey you complete, and include it with your Graduation Application Packet.
   1. Everyone:
      • "Survey of Undergraduate Degree Candidates"
   2. Majors with Education Option:
      • Professional Education "Technology Questionnaire"
      • Professional Education "Survey of Undergraduate Degree Candidates"
   
   
4. Walk Through Form
   This can be downloaded as a [PDF]. If you are having trouble viewing/printing this file, please visit the Downloads page for help.
   This must be completed if you are unable to attend your graduation ceremony and choose to "walk through" the ceremony in the prior semester. Please print, complete, and then submit this form to Sheila Adkins. Please see the Contact Us page for her current contact information.
